Pain Management Doctor - APPOINTMENTS

Before the appointment:

Please fill out the following questionaire and hand carry all MRI, X-ray films to your consultation appointment. All MRIs, X-rays will be returned at the completion of the appointment. Our office does not store any MRI or X-ray.

General Appointment Information:

All appointments are scheduled after getting authorization from insurances. There is no walk-in appointments since Dr. Chan is a Pain Specialist. The types of appointments are:

• Consultation services ( initial visit )
• Electrodiagnostic studies / nerve conduction studies ( EMG/NCS )
• Epidural injections - three appointments are scheduled including pre-op visit at the office, epidural injection at the hospital outpatient surgery center, and post-op at the office
• Acupuncture / accupuncture appointments
• Recheck appointments

Insurances Accepted:

Dr. Chan has joined many of the Workers Comp MPN (Medical Provider Network). Our office accepts most worker's compensation insurances, AllCare IPA, Blue Cross PPO, Central Valley Medical Group (CVMG), Medicare, Sutter Gould Medical Group. Dr. Chan is also contracted with Kaiser Permanente Medical Group. Please call our office to get more information if you have question about a particular insurance. The office also accepts cash pay patients.
